New Patient Intake Forms and Office Policies

Welcome to Meridian Health Solutions!

Before your first appointment, please complete the Intake & Consent Forms emailed to you directly via JaneApp (our secure portal), or by creating an account with JaneApp, which allows you to complete the Intake Forms, view your schedule, access requested receipts and change your contact information. Self-scheduling is NOT available.

For IN-PERSON appointments only, if you would prefer filling out paper forms, please find your practitioner's name from the list below and click on "these forms" to print the PDF version. Remember to bring the forms with you for your first visit, or email the PDF to info@meridianhealthnc.com before your appointment day. Please note, emailing your forms to our front office is not on a secure medical portal. Make sure you’ve read our Office Policies and Consent for your treatment, payment, and privacy considerations. It is important that you understand them and that we have your informed consent to treat you.
